Aged Care Advisory and Placement Services
Aged Care Connect is an aged care advisory and placement
service.
We can help you find assisted-living, low care (hostel), ageing-in-place,
or high care (nursing home) including dementia specific aged care for your
loved one.
We have proven skills and experience in matching your family needs to appropriate
aged care alternatives.
We are kept regularly informed about availability and wait list status by
aged care providers. We have built trusted relationships with aged care
providers, but we also remain independent and not aligned in any way. You
will receive totally unbiased information about the available residential
care alternatives.
Outline of our aged care placement services:
(1) Selecting the right facility
Don't waste your time looking at facilities which are
not suitable to your family needs. Rely on us to gather information about
care needs, location preferences, financial situation and personal factors
relevant to the placement.
The collection of this information is completed with an easy to use request
form that is emailed to you.
In return, you will receive a list of appropriate homes recommended by an
experienced placement consultant.

(2) Completing the placement
We will complete and submit all the necessary application
paperwork, including the Part A document, any application paperwork required
by the facility and the 'Request for an Assets Assessment' booklet.
After submitting the applications we work with the admission coordinators
to secure the placement and keep you updated with applications for placement
status.

(3) Family information session
The family information session provides current information, strategies
and insights about residential care placement.
The family information session is an excellent start for families with time
and energy to be personally involved with the activities needed to complete
the placement.
This service consists of a private consultation with the family for around
two hours, usually conducted in the comfort and privacy of the family home.
The placement consultant will describe the steps to be completed, make the
family aware of many useful hints and guidelines and answer any questions
that the family may have about residential care.
The placement consultant is able to provide the family with all of the current
rules and effective strategies relating to residential care placement. The
fee for the information session is payable on the day of the consultation.
The benefit to be achieved are that family members make an excellent start
based upon professional knowledge and experience and the cost of the service
is affordable.

(4) Accommodation Bond Negotiation
This bond negotiation service is designed
for families who have already selected the aged care home but have not finalised
the accommodation bond or extra service fees with the aged care provider.
In the event that the care recipient has been placed into an aged care home
prior to financial arrangements being agreed with the aged care provider,
the family can employ our placement consultants to negotiate the accommodation
bond or extra service fees with the aged care provider.
By using our experienced placement consultants, you can achieve a better
financial arrangement for your family.
An experienced placement consultant is aware of current strategies that
have been proven effective when negotiating fees with the aged care provider.
The benefit of this service is that it assists the family to dramatically
reduce the cost of residential care.

(5) Personal Placement Service
Our personal placement service has been designed for
families who prefer to have complete assistance available from start to
completion of the placement process, and includes:
- a family consultation, to gather requirements and to ensure that
the family is completely aware of the current rules and regulations
affecting residential care
- preparation of facility shortlist
- arrangement of facility visits to meet the care team and assess the
quality of care and lifestyle services
- negotiation of residential care bonds and related fees
- completion of all application and admission paperwork
The fee for our personal placement service is paid in two parts, a deposit
payment after the family consultation and final payment after the placement
has been completed.
