Making the applications
Application process
Most people who have been through the steps of aged care placement know
that that the paperwork can be comprehensive.
Many aged care homes use the forms suggested by the Commonwealth but some
use thier own application forms.
As well as the application forms, there is the Centrelink / DVA "Request
for Assest Assessment". This form is a booklet, contains 28 pages,
over 80 questions, and needs its own "information booklet" of
7 pages describing how to complete the first booklet.

Also have you completed the necessary Power of Attorney or Appointment of
a Nominee forms?
Knowing what to do, and how to do it right the first time, will help you
avoid delays in getting the paperwork processed.
