Making the applications

Application process

Most people who have been through the process of obtaining a residential care placement know that that the paperwork can be comprehensive (and exhausting).

Many aged care homes use the generic forms suggested by the Commonwealth government but some prefer to use thier own application forms.

As well as the obligatory application form, there is the Centrelink / DVA "Request for Assest Assessment". This form is really a booklet and contains 27 pages, over 80 complex questions and its own "information booklet" of 7 pages describing how to complete the first booklet.

Also have you completed the necessary Power of Attorney or Appointment of a Nominee forms?

Knowing what to do, and how to do it right the first time, will help you avoid long delays in getting the paperwork processed.

Aged Care Connect can help you by:

Accurately completing all forms and assessments and lodging them with each selected residential care facility and other relevant organisations.

Contacting the selected residential care facility to confirm receipt and assist with early acceptance.

Andrea has become somewhat of an expert in the application for guardianship rules - so if you have any questions in that area, contact Andrea.

We take that little extra effort to dot the i's and cross the t's to make sure our family clients get the best result possible.

Let our experienced aged care placement consultants make sure that all the paperwork is completed right the first time
