Eligibility:
- People over the age of 65, who are frail and requiring assessment
for future care planning, including determining eligibility for Commonwealth
funded aged care services; such as permanent residential care, residential
respite or community care packages. See Attachment 2
- Aboriginal and Torres Strait Islander people aged over 50 years.
- People aged under 65 who are ineligible for Department of Human Services
funding/services (DHS Intake 1800 783 783).
Note - Please notify Gateway lead via email when referral for a person under
65 years of age has been recieved and processed. NB: Requests for residential
respite care extensions do not require new assessment - these requests go
to the Assistant Manager or Senior Delegate, Gateway.

The Grampians Aged Care Assessment Service (Grampians ACAS) with Ballarat
Health Services is a team of health professionals with expertise in health
care and community services for frail, aged people and/or younger people
with disabilities.
The team draws upon the skills of geriatricians, community nurses, social
workers and a range of allied health professionals including physiotherapists,
occupational therapists and speech pathologists.
Grampians ACAS is responsible for comprehensively assessing the needs of
frail older people and when required younger people with disabilities to
facilitate access to available care services appropriate to their needs.
On receiving a referral, initial contact is made to arrange for an assessment
to take place. This is undertaken preferably in the clients own home to
discuss needs and identify a range of options.
An outcome of that assessment may be a finding that a person is eligible
for residential aged care, Community Aged Care Package (CACP), Extended
Aged Care in the Home Package (EACH/EACHD) and/or residential respite.
Grampians ACAS is the only local agency authorised by the Commonwealth to
approve clients for these services.
Grampians ACAS staff can provide information and referral advice and will
work closely with individuals, their carers and their GP / service providers
to ensure the most appropriate services are put in place.
Eligibility Frail aged individuals with complex care needs. Younger
people with a disability with high and/or complex needs for eligibility
for Commonwealth funded residential care.
Waiting List Grampians ACAS clients are registered and prioritized
according to individual needs.

The North West Aged Care Assessment Service (ACAS) provides assessment,
information and advice to clients and their carers to help them make informed
decisions regarding individual care needs.
The service supports people in their own homes and communities by linking
them to health services which promote, as far as possible, rehabilitation
and restoration of function. The ACAS can also refer people to community
services which will maximise their independence and safety.
The ACAS has Commonwealth Government delegation responsibility to assess
eligibility for nursing homes, hostels and Case Management Services if required.

The Mildura Aged Care Assessment Service (ACAS) aims to ensure that
frail older people, and their carers, gain access to available services
appropriate to their needs, and to improve coordination of aged care services
with other health and community support services.
Our Service Assists when: a person identifies a need to discuss plans for
future care concerns that a frail older person is unable to cope at home
due to physical or mental deterioration carers have a need for short respite
periods a person requires increased support in order to remain independent
in their home.

The Warrnambool Aged Care Assessment Services (A.C.A.S.) is an independent
team which assists older people and their carers to identify what kind of
care will best meet their needs.
Assessment teams are multi-disciplinary and can include health professionals
such as medical officers, social workers, psychologists, nurses, occupational
therapists and physiotherapists.
They conduct comprehensive assessments for those needing community services
or aged care residential services. They assess for all potential restorative
and care options.
For an older person to access Australian Government funded residential care,
residential respite, Community Aged Care Packages (CACPs), Extended Aged
Care in Home (EACH) packages or flexible care, they must first be assessed
as eligible by an A.C.A.S.

Thanks. Mum has been
in the aged care facility now for nearly 2 weeks, time goes quickly!! Im still
very pleased with the facility, the staff are very nice, and it is good to be
able to visit more regularly as it in only 10 min from home. The single room
is just what i was hoping for, and we have started making it more homely with
a nice coloured doona and pictures, and some of mums trinkets put in place.
My eldest son called in the other day and he was pleased and said it felt bright
and friendly with the nice central gardens in the middle it made him happy knowing
his grandma was in a much nicer place than the previous aged care home. Cheers

help in relation to finding a suitable home for my Mum and Dad. Your efforts

Yesterday I received
a call from the low care facility in the area of which we were looking to get
my Father in. My husband and I went to have a look and absolutely loved what
we saw. It was exactly what we wanted in a secure dementia room. We immediately
took the room and on Monday I go back there to sign all the paperwork, then
the following day, we can move our Father in. They did mention that they received
all of the necessary paperwork from you guys, which was a blessing to both my
husband and I. I would just like to thank all the staff there in taking off
the added pressure of doing all the 'dirty' work for us. We are forever grateful
to you and your staff. The fee that you charge is more than worth the work and
the time that you do. I am in shock of how quick things moved along. As we are
at an age where most of our friends are in the same boat, I have been giving

say Thank you. I can not express just how much I appreciate all you have done
for us. I am extremely close to my wonderful mother and it has been heartbreaking
to have watched her suffer the way she has. Finally she is showing improvement
and the care and staff are so terrific at .... All thanks to you we can now
