Facility Applications and Placement

Our facility application service is designed to assist families to identify at least six facilities which are suitable based upon the family needs and preferences, and to make applications to at least six aged care homes. We take the quesswork out of the all important task of selecting the right aged care home for your loved one, and we help to eliminate the paperwork required for you to complete.

After gathering information about the care recipient needs, location preferences, financial situation and social aspects; our experienced placement team will use their current knowledge of the local aged care homes including important information about waiting list status to provide you with a shortlist of appropriate aged care facilities.

We will provide you with a facility shortlist, help complete all of the important paperwork, including the "request for asset assessment" and "Application for Entry to an Aged Care Home - Part A and Part B". We will submit applications for you and monitor those applications until you are offered a placement.

The major benefit of this service is that the family can be assured that the aged care homes selected have been matched to the family needs, you have minimal paperwork to complete, and you have an experienced placement consultant monitoring the process of the applications.

We follow the following steps:

Contact us by using our simple form and request a placement consultant to contact you.
We will email, fax, or post you a list of simple questions about your family needs and preferences. These will include care needs, location preferences, financial and personal preferences.
Simply email, fax, or post your responses and a copy of the Aged Care Client Record (ACCR) to our placement team, who will immediately start work on your behalf to research current availability in your preferred locations.
The list of recommended aged care facilities will contain at least six aged care homes which are appropriate to your family needs. The list will contain links to the aged care homes so that you can undertake further contact with the homes to make the applications. You will have managed to reduce the liklihood of contacting aged care homes that are not applicable.
We will complete any other paperwork required prior to making applications, such as the "request for asset assessment" and "Application for Entry to an Aged Care Home - Part A and Part B".
We will submit applications for you and monitor those applications until you are offered a placement.
